Patient Rights and Responsibilities

Everyone who comes to Immediate Care Center of Westmont has the rights described below, at both our Cass Avenue clinic and our clinic inside Jewel-Osco.

Your rights

• To be treated with respect, dignity and consideration, regardless of who you are.
• To receive care without discrimination.
• To know the name and role of the people caring for you.
• To have your condition, treatment options, and expected results explained in language you understand, and to have an interpreter if you need one.
• To take part in decisions about your care, and to refuse any treatment, and to be told what refusing may mean for your health.
• To privacy during your visit, and to have your health information kept confidential in line with state and federal law.
• To see and request a copy of your medical record, and to ask for corrections to it.
• To know what your visit is expected to cost, and to receive an itemised bill you can ask questions about.
• To be told if you need care we do not provide, and to be directed to somewhere that does.
• To raise a concern or make a complaint without it affecting the care you receive.

Your responsibilities

• To give us complete and accurate information about your health, medicines, allergies and history, as best you can.
• To tell us if you do not understand something, or if you cannot follow the plan we discussed.
• To treat our staff and other patients with respect.
• To provide your insurance information and to meet your financial obligations for care.
• To keep or cancel appointments where possible.
