Downers Grove North and South athletes: the preseason exam
We complete sports physicals for Downers Grove North and Downers Grove South athletes before each season. The American Academy of Pediatrics (AAP) suggests a sports physical at least 6 to 8 weeks before practice begins, and it recommends one for all middle school through college-age kids, whether or not they play on a team.
Heart health: questions about chest pain, fainting and family heart history, plus blood pressure and a heart exam
Mental health: the AAP suggests asking about stress, mood, anxiety and attention in private
Bones and joints: old sprains, fractures and how well each joint moves
Concussions: no return to sports until a health professional gives clearance
For female athletes: menstrual health, nutrition and bone health
The AAP's history form comes in English and Spanish, and some of our staff speak Spanish. Bring the form your school or league uses. The AAP prefers that your pediatrician do the exam when possible; if we do it, share the results with them. More on our sports physicals page.

Some Downers Grove, IL students and workers need more than an exam. A school, college program or employer may ask for a TB test, and the school form asks about vaccines. IDPH's 2026 to 2027 rules call for one dose of Tdap on entering sixth grade, one meningococcal dose on or after age 11 for grades 6 through 11, and two meningococcal doses for twelfth grade.
Tell the front desk everything your form lists when you check in, and ask whether a TB test needs a return visit to read. Some vaccines may be available here, so call (630) 324-6825 to confirm before you come for a shot.

Can a Downers Grove North or South athlete come in on a weekend for a sports form?
Yes. Both Westmont, IL clinics are open Saturday and Sunday from 9 AM to 5 PM. Bring the school's sports form with the history pages filled out, plus glasses or contacts for the vision check. A parent or guardian should come along for athletes under 18.
Can I get a TB test and a physical at the same visit?
Usually, yes. Tell us at check-in that your form needs both. Some TB tests need a second visit to read the result, so ask about timing before your deadline. Bring the school, program or employer form so the clinician can record everything.
Does my District 58 kindergartner need a physical before the first day?
Illinois requires the exam within one year before a child enters kindergarten or first grade. IDPH sets the proof deadline before October 15, but your school may want it earlier, so check the district's date. Come in early in the summer if shots are due.
Can I get the sports history form in Spanish for my Downers Grove, IL athlete?
The AAP's preparticipation history form is available in English and Spanish. Your school may use its own form, so ask the athletic office. Some of our staff speak Spanish and can help with check-in. Whichever form you use, fill it out at home with your athlete so the clinician can go over the answers with you.

“Everything went as smooth as can be with scheduling my virtual appointment. I also called the staff a few times to ask a few questions prior, and they couldn’t be any more friendly. Coming out of my appointment I feel less anxious as I feel they’ve provided the appropriate care that I need. I 100% recommend this provider.”
