School, Sports & Work Physicals for Clarendon Hills, IL
The Village of Clarendon Hills lists four school districts that serve its addresses: Community Consolidated School District 181, Maercker School District 60, Hinsdale Township High School District 86 and Community Unit School District 201. Your street decides which one. Whatever the district, the required school exam goes on the same state form.
The Illinois Department of Public Health (IDPH) calls it the Certificate of Child Health Examination. A required exam must be done within one year before entering kindergarten or first grade, and again on entering sixth and ninth grade. It is also needed before preschool and whenever a child enrolls in a new school. Proof is due before October 15 of that school year, though schools may ask sooner.
Since 2024, the form also has a space to note whether a social-emotional and developmental screening was done. IDPH says a child is not kept out of school if the provider leaves that part blank. Details are on our school physicals page.

My child just had a well-child visit. Do we still need a sports physical?
Maybe not. The AAP suggests asking for the sports form during the yearly well-child visit, so your pediatrician may be able to sign it. If that visit has passed and the form is still blank, we can do the sports physical and you can share the results with your pediatrician.
“Everything went as smooth as can be with scheduling my virtual appointment. I also called the staff a few times to ask a few questions prior, and they couldn’t be any more friendly. Coming out of my appointment I feel less anxious as I feel they’ve provided the appropriate care that I need. I 100% recommend this provider.”
