Club and school teams: getting the sports form right

We see Western Springs, IL athletes on school teams and club teams. The American Academy of Pediatrics (AAP) says a sports physical looks at heart health, mental wellness, bones and joints, and past injuries, and for female athletes, menstrual health and nutrition. The AAP suggests the pediatrician ask about stress and mood in private.

The AAP also notes that most kids are cleared without restriction. When a concern comes up, such as fainting with exercise or a recent concussion, it usually means follow-up first. The AAP prefers that the pediatrician do the exam when possible; if we do it, share the results with your child's doctor. See sports physicals and sports physical vs. regular physical.

If your club team has no form of its own, the AAP offers a free medical eligibility form you can print and bring.

The Illinois Department of Public Health (IDPH) asks for a school health exam at preschool entry, within one year before kindergarten or first grade, on entering sixth grade, on entering ninth grade and whenever a child changes schools. Proof is due before October 15, though schools can ask sooner.

Since 2024 the state form includes a box for whether a social-emotional and developmental screening was done; IDPH says a child is not kept out of school if it is left blank. IDPH's 2026 to 2027 vaccine rules add Tdap for sixth grade and meningococcal vaccine for grades 6 through 12, and the CDC schedule lists the same shots at ages 11 to 12. Bring the vaccine record to the school physical.

Camps send their own health forms; bring the parent section filled in, plus medicines and allergies. Teens starting a first job may need a non-DOT work physical; we do not do commercial driver (CDL) exams. The Cook County Department of Public Health recommends a flu vaccine for everyone 6 months and older, so a fall physical is a good time to ask about immunizations. Call ahead to confirm stock.

Our team is led by board-certified physicians. Neighbors use the same clinics; see the La Grange, IL physicals page and the Hinsdale, IL physicals page. Book online to set a time.

Common questions

Do Western Springs, IL club teams use a different form than school teams?

Often, yes. Schools and clubs each choose their own sports form, and the required school exam uses the Illinois state form. Ask each coach or athletic office which form they want. If a club has none, the AAP offers a free medical eligibility form you can bring.

How early can a Western Springs, IL family book a summer physical?

For kindergarten or first grade, IDPH says the exam must be within one year before school starts, so early summer works well. For sports, the AAP suggests at least 6 to 8 weeks before the season. Many schools and leagues accept an exam from within about the past year, so check each form's rules.

Can a Western Springs, IL teen ride Metra to a physical without a parent?

For anyone under 18, a parent or legal guardian should come to the visit. If that is not possible, call (630) 324-6825 ahead of time and ask what written permission we need. The parent should also fill in the health history pages before the visit.

Why does my Western Springs, IL athlete's form ask about relatives' heart problems?

Some heart conditions run in families and can raise risk during hard exercise. The AAP asks families to answer those questions honestly and completely. If a relative had heart disease or died suddenly at a young age, tell the clinician, who may suggest follow-up before full clearance.
